Caregiver Role Guidance:

Foster Parent - Select this option if you are a licensed foster parent or are applying to become a licensed foster parent through Idaho Department of Health and Welfare.

Kinship Caregiver - Select this option if you are caring for a child in foster care with whom you have a significant family-like relationship and are completing training to become a licensed kinship caregiver through the Idaho Department of Health and Welfare.

Therapeutic Foster Care - Select this option if you are a licensed therapeutic foster parent or applying to become a therapeutic foster parent through Modern Adoption Inc.

DHW Staff - Select this option if you are an employee of the Idaho Department of Health and Welfare.

Other Staff - Select this option if you are an employee of a private child-placing agency (case manager, supervisor, licensing worker, clinician, trainer, support staff, etc.) and are completing staff training.
